I thought that Gyokko will be top-1. Can't really call Douma evil - he has no concept of empathy at the first place. Kinda like Akaza's and Kokushibo's situation, where their circumstances justify them in some way. Gyokko, Muzan and Enmu knew how much pain they cause and they loved every bit of it.

Douma definitely is not the most evil, most psychotic and mess yeah, but not when it comes to evil. Evil people enjoy to cause havoc and suffer, or they have a evil goal they want to achieve. Douma is none of that, he has no sense of malice, no ill intentions behind him harming people, 0 goal in his life. Only the belief he is saving humans. Out of all the demons, only Douma kill out of MERCY, he wants nothing but make people happy. Compare that to Gyokko and Enmu who enjoy to see humans suffer, or Hantengu who pretend to be the victim. Douma is 100% less evil.

Speaking from a character writing standpoint, i think Enmu is quite underdeveloped compared to the other demons because, unlike most of them, he didn't get a backstory in the main story, and the information about his human past in the extra material was rather less detailed and open to interpretation.

I know his backstory tho- When Enmu was a human, he couldn’t tell the difference between dreams and reality and confused everyone around him. If that was it, he would have just been another weird kid. However, the oddness of his personality would come to stand out as he got older. He used hypnosis therapy to convince sick people with limited time, that they were cured, then took away their hopes by revealing his lies. He was an evil human with no morals, but that personality hasn’t really changed since becoming a demon…
