If they aren't taking 2 of the same abilities, and dying, then yep it's a player issue, unless you're in mythic and they are super undergeared etc.

Im worried that im going to screw up the swaps... does anyone know if its possible to setup WA or DBM so that the boss abilities make a noise/ping? I would love to set up an audio reminder which pings when these abilities are about to occur or are occurring. I find it hard to keep track of the abilities whilst dodging bad and everything else we have to keep an eye on. If you have any other tips for a relative new tank to help with this... I'd love to hear them!

it is different AND it will overlap with previous one on HC

Yes, you can get 2 slashes back to back etc on all so you just need to pay attention =)
