It was a form of torture and execution as seen with the beheaded people outside. It was designed to make someone beg for death and then get beheaded. If I remember correctly, Midra encountered the three fingers/frenzied flame and he became a sort of host to the frenzy similar to how we can in elden ring. Because he's been touched by the three fingers, he got stabbed with the sword as a form of punishment and to contain the frenzy, until we attack him in the dlc and now he can be as mad as he wants
