How to create task on same record after someone reply to the mail

Hi Sir, I have simillar requirement for this. But for the 'Replye' type. We are sending email to user table is 'sc_req_item' and we have to wait until email get response and after the response wee need to create sc_task on that 'sc_req_item' same record. How can I achieve this?? Please help me
