Seems fine to me. Doesn't seem like a huge decline in quality in the "live" vs edited format, beyond what you already identify. Did feel a bit sparser and shorter than some, but I put that on the contents of the Scrubs episode as opposed to the format. Thanks Doc.

The jumping to cancer on Dr Cox’s part I think was intentional to scare the patient. This is only the 2nd episode of the series so we don’t know the character that well, but I do think it fits into what we know about him so far. He saw a young guy not taking the situation seriously enough so he comes in with the cold shower.

Regarding the whole “expecting cancer” thing, what I interpreted from that scene is they think cancer is unlikely as well but they’re just doing due diligence in ruling it out.

I think the way the episode plays it is kind of funny, because nothing the characters actually do contradicts your take on it (which is my interpretation as well), but the overall writing and tone of the storyline is definitely overfocused on the scare factor of cancer. It's like if the characters were real and they were doing a documentary but the editors thought a cancer scare would interest the audience more than the detailed investigation, it would come off the same way :)

Always stop what I'm doing once I see a Dr Hope video drop! Love your Scrubs reactions doc! Honestly the quality was completely fine. I noticed some differences in appearance, but at no point did i feel anything was lacking. I'd say if your current setup allows you to more easily make videos then it looks like it's working really well for you, and I'm happy because we get more great content!

I encountered someone who wouldn’t admit to smoking to her doctor, because she “was tired of the lecture.” I had to tell her that regardless, her doctor needs to know her smoking status. And if he/she didn’t do SOME kind of anti-smoking lecture - even just a reminder of the health risks - they wouldn’t be a very good doctor. My doctors push me to lose weight. It’s not “body shaming.” It’s trying to encourage wellness and a long and healthy life.

Depends on the context. I had an impacted finger from playing basketball. My doctor ignored the finger and gave me a lecture on weight loss. End result: I didnt go back to the doctor for seven years. Every time I saw her she told me I would be dead within five years. After fifteen years of treating me as fat first and a person second, she died, and twenty years later I'm still here. No amount of nagging can substitute for good bedside manner.

@@andrewgrant6516yep, same for my mum. Last two times she went to GP they ignored the reason she went in and instead started lecturing her on smoking. She's nervous of doctors so rarely goes to GP and that kind of attitude doesn't help. For anyone saying it's the Drs job, they could wait until near the end of the consultation instead of at the start which will only get the patient defensive as well as feeling like the problem they went in with is being ignored (and sadly sometimes is).

I dunno, I think it depends. My GP has had a very serious conversation about my weight with me. We talked options, and talked about challenges to what I face. And that conversation was over. Just like how we don't "check in" on my OCD every appointment, we don't check in on my weight. I know, she knows I know. Doesn't make her a bad GP, it makes her not the pimary person able to make that change. If I have a health issue related to weight, it needs to be discussed. When I have health issues unrelated to weight, bringing it up is a distraction and a waste of clinical/hospital time. Summary Care Records are brilliant and a good medical professional can use them to know what has and hasn't been discussed.

@@CTM418 if you're fat you're not healthy, period.there is no question around this topic. Smoking at all is not healthy and drinking alcohol regularly also isnt great. If you come in with a broken finger and you're morbidly obese your doctor should treat the problem at hand immediately but the actually life threatening thing is most likely still the weight/fat percentage and not a cracked finger. So yea it might not feel good to hear but the doctor has some point

Always great seeing your Scrubs reactions; I tend to avoid a lot of your reactions to contemporary stuff for fear of spoilers, and Scrubs is very react-worthy. Though it did seem short, like there was plenty episode left to discuss, but I imagine that might've been because there wasn't much to comment on in this particular episode. I'd say the new set recording style worked; I didn't really notice a difference until you brought it up. That said, I'd miss the demonstrations - like the drawings and skeleton - if this style continued strictly as-is. But, of course, your commentary is the big draw, so you do what you need to, mate.

I work in the addiction area, and I don't think I've seen a medical drama look at treating patients who are drug dependent. Scrubs did have an ep about a patient faking an illness to score opiates, but that's different from having to manage a patient's addiction while treating another condition.

Took my husband a good year. During that year he really missed his good friends "Nico and Tina". Their absence left a big void that was only filled a good year later. Now he is 14 years without cigarettes and cant imagine he ever needed them. He even thinks the smoke is disgusting, when he encounters smokers. It was really hard the first year. I almost asked him to start smoking again, because he was so grumpy, depressed and sometimes plain agressive...😅. Glad he (and to a lesser extend me) did not give up. He quit cold turkey, after multiple times of using patches, gum etc. Those things did not work for him, he needed to have all or nothing. Smoked around 30 a day. Keep up the good work. It can be done and your body and mind eventually overcome the addiction, it just takes a long time

It's always fun to see that _Scrubs,_ a show that takes such _outrageously_ silly comedic license with so many things, is nevertheless rated so consistently as one of the most accurate shows by medical professionals. Accurate to the _experience_ of being a young doctor or nurse (or janitor), that is - not necessarily accurate to specific medical details or treatment practices. The general refrain is almost always along the lines of, "Yeah, that's not exactly how that works, and this one thing isn't really how we do it, and probably wasn't the way it was done, back then either - but *_that_* is _exactly_ how it feels when you're in that situation… and you _will_ be in that situation, sooner or later."

I'm glad to see you talk about the struggle of wishing that patients would change their habits. So many doctors (particularly here in the US, and particularly for fat patients) are extremely judgemental and dismissive of such patients. And I get how frustrating it could be from the doctor's point of view. But the truth is that there are so many barriers and challenges to pursuing health that might not be within the patient's control, e.g. mental health problems, financial problems, lack of access to healthy food/movement, lack of time or energy, executive dysfunction, etc. And shaming almost never helps anyone. Shaming is far more likely to cause the sort of distress that will cause someone to retreat further into whatever they're being shamed about. Heck, the CDC's model of population-level health outcomes has individual lifestyle choices only accounting for 20% of the outcomes. The rest are things like genetics, socioeconomic status, quality of accessible healthcare, etc. I wish more doctors would lean towards empathy and compassion rather than judgment.

I think Dr. Cox with the defibrillator at the beginning isn't meant to be realistic not just because they wanted a joke but because *in the universe of the show* it's not what's actually happening - it's JD re-interpreting his surroundings as if he was in a "I'm so competent now" musical montage. Just like we can think the patient's feet and Turk's exam in that scene aren't actually moving in time with the music. And I think the fact the resuscitation scene is so farcical highlights the self-aware irony of that whole sequence - JD *feels* super-competent but he's still very inexperienced to the point his visual imagination of hospital scenes is very sketchy and drawn more from inaccurate TV medical dramas than personal experience. I also wonder if it would be in-character for Dr. Cox to have ordered the tests and talked about it being cancer not because the probabilities justified it medically but in order to "scare the patient straight" in a more subtle but maybe effective way than JD's straightforward approach. I'm kind of in two minds about it if so - my general intuition is that playing such games is a dickish move and I can easily imagine it being pretty bad if every doctor did this, to the point of undermining the trust of smokers in the medical field ("the moment they find out I smoke it becomes all about that, they're not trying to help with the problems I went to see them about"). On the other hand it does seem like this experience could be a very concrete way for a smoker to internalize the reality of their cancer risk, even maybe this guy on a subconscious level. Either way I bet Dr. Cox would give zero fucks about my intuition that it's a dickish move. Whatever the writers intended it could be a decent headcanon for restoring your suspension of disbelief on this plot point.

Scrubs remains one of my favourite episodic programmes. Dr Cox is, without doubt, one of the best characters in an episodic drama, and was always used to great effect to convey great wisdom to the audience. Scrubs want just a great medical comedy-drama; it was like good children's television programme (conveys useful information and morals) but created for adults.

I can totally relate to the cannula thing. As a student nurse, I was really nervous about doing things like sub-cut injections and putting in cannulas and taking blood. So far I have done a couple cannulas, and one of them the patient literally pulled it out before I had managed to get the dressing on and the doctor had to put it in, she was nice and still signed it off for me.

Smoking IS a form of suicide. It can take a long time, but you eventually get there, with possibly all sorts of physical travail along the way. I know this as at 74, I have smoked consistently for 58 years and, though I hardly ever see my GP, clearly my body, particularly my lungs, is now telling me it's getting worn out. It's partly a case of addiction and partly a case of how you view life that I continue. The thought of how much this has cost me in financial terms is staggering. A good and telling video, Dr Ed. So a pleural effusion would show up on a chext x-ray and would not usually require a CT scan unless the effusion is very large, in which case we would want to do a CT but that would be done after draininage. (Check the British Thoracic Society Guidelines). However, a young patient with multilobar pneumonia, even one that smokes is quite unexpected. I might request an HRCT or more likely just a standard CT Thorax depending on the presentation of the patient.

Scrubs holds a very special place in my heart, I have all the episodes of scrubs on my hard drive, yes even s9!, Yet I still bought s1-3 on dvd when I chanced upon them in a charity shop, purely for the extra scenes and outtakes. It's was an awesome show! So Funny and serious at the same time, very heart warming. The steak night episode always sticks in my mind as one of the greatest drama episodes and almost makes me cry every time I watch it and I've seen it about 20 times! "We can only hope that our last memory is a good one!", Looking forward to the review of that one and the rest of course. Could talk about scrubs for days. Edit: how did I forget you reviewed this one last year and I even watched it... Duh!

I work with med students as a patient roleplayer, and honestly us actors had to listen to the smoking lecture SO many times, we asked them to stop adding it if it was irrelevant. Don't get me wrong, it wasn't because we took offense or anything (though as a vehement anti-smoker, the patronising tone got very grating), it was more because the med students would just get stuck on that one topic and not move on. In exams, they only have 7-20mins depending on the exam (20mins tend to be two parts or psych) so it was in their interest too. But yeah, the amount who would forget to ask in the younger years, or assume "no" meant "never", was kinda scary....

In both this episode and all over the medical genre, there's a running trope that doctors always assume patients are lying about how much they drink/smoke/eat/have sex/whatever their vice is, and it's always bothered me. It seems doctors in real life operate on the same judgment because they never seem to fully believe my answers about things. It can get really frustrating to trust doctors enough to give truthful answers when this judgment is always present ):

Love your Scrubs videos! However, I would link to comment on one thing - don't say he's too young for cancer. You can get cancer at any age - in the US, 90k AYAs (15-39) get a cancer diagnosis every year. It looks like for the UK, it's 2400 cases in 15-24 year olds, then 162 per 100k in 25-49 (If I did the math right, that comes out to 108k cases per year?) - so add them up and it's 110k cases per year. Please don't try to marginalize the AYA population. If you had said they shouldn't have told the patient they were considering cancer, sure - that is a good thing, but nobody is too young for cancer.

I've worked for over 30 years in various hospitals. I've seen the same patients come in every few weeks or years Each time they are a little worse until that last time. Often, they do not follow the advice of medical staff. You just have to move on.
