It always seemed to me like the song and video are about someone who gets violent when he's drunk. His character was passed out on the street at the start of the video, there's a reference to being "hooked on the bottle," and the reputation he's amassed while not in his right mind follows him wherever he goes. Also, the title suggests that this is self-inflicted, and not a stigma that's been wrongfully inflicted on him by society.
