Depends on your level of understanding, and hers too, to determine if you have to say you are sorry or not. If you both understood love, you wouldn't have to say you are sorry, but you could if you wanted, but she would already know.. She wouldn't need to require you to say it because she knows you are sorry, therefore when you are sorry for real she will know already, and already know you will make sure it don't happen again because you for yourself are truly sorry. Note: but you won't find anybody who understands enough to never require their spouse to say it, most likely never find anyone that intelligent, or faithful, trusting, or patient.
