Maybe pay attention when he's talking. He clearly said he has other accounts that craft food and weapons/armour, so on his other accounts he has focus and premium. On his refining account he doesn't have premium because refining doesn't need focus it's still profitable without it.

That minus 6.5% is tiny when you mass refine on a large scale. It only matters if you're small scale refining. He made 5mil profit so 6.5% is an extremely tiny loss.
