Clearly it was a fan trying to throw a gift on stage, he/she I'm sure did NOT mean for it to hit Regina, especially in the face, they were probably mortified that they'd accidentally done it, and I think Regina clearly noticed this is as well, which is why she didn't let security throw them out.

omg whoa seeing regina get hit in the face with anything is really horrifying... especially since everyone in the crowd gasped and squealed with shock. The audience's reaction was actually really heart-warming because it showed how concerned and protective everyone is of Regina. That's love. It definitely was not a wise plan to throw it right at her in the middle of a song, but I do sympathize for the girl who did it. I brought Regina a gift when I saw her, so I understand the desperation.
