My tip: start with the musculocutaneous nerve to prevent that some patients might move or pull away their forearm while you are hydrodissecting the other nerves.

So my frustration is that neither supraclavicular nor axillary block provide a reliable block for ulnar nerve transposition. The entire arm is numb except for the incision site. I’m assuming I’m missing the median cutaneous nerve. Is this nerve usually blocked with axillary block? If not, what additional block can I do for post op analgesia? Or is it just my technical shortcomings? It def could be my technique, although my blocks usually work for other upper extremity procedures.

Hi there, it sounds to me like you’re missing the intercostobrachial +/- medial cutaneous nerve of the arm. This is super common with axillary brachial plexus blocks (so it’s NOT your technical ability 😊). And the good news is there’s an easy fix for that. Why is it OK to inject local anesthetic in the sheath at this level, but should you stay out of it at the supraclavicular level?
