Saline implants are a bit safer, because if they leak, it's only saline. The problem is the lack of real options. The patient would not want them to only take the implants out, which seems to be the safe option at this point. And I guess if there is a critical medical issue, practically every surgeon can get the implants out. So the patient has at least breasts she seems to like at this point ...

i've seen episodes where the implant has flipped inside the breast pocket. she may have lost some of the saline in the other breast. that gets absorbed by your body. it seems odd she didn't notice after that surgery the size difference. so maybe over time it leaked internally and became smaller.
