We use volume trigger here in pakistan. We set the volume trigger of 2L for pediatrics and 3L for adults ....

Question about auto PEEP and which trigger sensitivity to use. If a patient develops auto PEEP and is on pressure trigger, then the patient must pull past the auto PEEP first before getting to the trigger setting. So if they have a brief period, even one quick breath, of auto PEEP, then they can get thrown off their ability to trigger. With flow trigger it is my understanding that their ability to trigger is not going to be more difficult. So I prefer flow trigger. But someone told me that I'm not correct with my thoughts on this. So I thought I'd throw it out here.

I'm not sure about having to pull pass the auto-peep, but I definitely see what you are saying and it makes sense. And yes, flow trigger is easier and definitely not affected by auto-peep. Let me look into this a bit more. I'm not sure if a pressure trigger of -2 equates to -2 presssure decrease from set baseline or from actual baseline. This may actually be something that varies across ventilators. Very interesting!!! Thanks for watching and throwing your thoughts out there.

We briefly ended our semester with mechanical vent but I want to be prepared for the upcoming semester when we go more in depth with ventilators. Hey Raya! Bias flow in regards to flow triggering is typically set around 5 lpm with a flow trigger set 1-2 lpm, which means when the ventilator recognizes 1-2 lpm missing from the original bias flow of 5lpm then a patient triggered breath is recognized and inspiration begins. Your bias flow must be greater than your flow trigger to function. There are other various discussions related to bias flow, HFOV for example. Let me know if this is not the bias flow your were referencing. Best wishes on your final tomorrow!

Hello! Flow is easier and typically preferred, but there is nothing saying you can't use pressure trigger. There's also nothing that says you can't use flow trigger when in pressure control. Not sure where that common thought process began. So whichever your patient tolerates is the best.

Never heard of volume trigger in practice. Seems dangerous, especially if compliance is poor and the patient is completely sedated. Not exactly something I'd want to attempt.

I've actually never seen it as an option, however Egan's references it on page 999 of the 11th edition. I don't know!
