It's not an add-on, simply go into settings -> interface -> "use raid style party frames" or something like that. Comes with a lot of customisation aswell.

It's most definitely 3k. Playing with a warlock you don't want to spec/use cyclone since it shares DR with fear. Also feral affinity allows for a stun when in prowl (stealth), as well as mana-reg and tactical positioning. You should check out some more druid videos on KZbin, literally every druid above 2.2k does this!
