I have installed a dozen of these and I've never seen a template card in the box. I've always just aligned everything by eye, but I get really mixed results, which is frustrating, which is why I looked up this video. I guess I will try to track down a template somehow.

It’s inside the paper manual. For me it was pages 5 and 10, you have to cut them out.

my false fronts were already off before i rec'd the tip out shelf so i can't trace the inside lines. what is the best way to proceed?

Hi Ryan! The best course of action would be to hold the false front where you plan on installing it and tracing the inside from there . You may need another set of hands to hold it in place while you trace!

I'm an experienced DIYer, and I must admit that this tip out tray is the most frustrating product I have ever installed. Personally, I will never buy/recommend this product ever again.
