Plot twist: All of the events in Season 4 are Victoria's imagination. She's never escaping the mental hospital in the first place and she's imagining the whole thing. Noticed in Season 4, most of the time she's always "winning" ? The moment when David shot her is the moment when she should've woken up from her imagination and the shot is this ending.
