does the 18.25" width is with the toy bar? I have a 17.5W nursery dresser, will this fit in there?

Yes, the pad is around 15 inch so your dresser would be fine even if the toy bar adapter sticks out a little bit.

Yes, we had them laying naked a few times and they were fine. They even fell asleep here and there when we changed their diapers.
