Honestly I agree, it's far too early. It's not until well into the last arc that they really start to understand each other. Although we never really do hear Utena call Anthy by her name, do we? It's nice to have this audio regardless.

I actually like it the first way better. The explanation given is good enough for me. She's distressed, and so she isn't thinking at all about what she's saying, and her subconscious desire to call her Anthy manifests itself briefly. This has always been one of my favorite moments of the series, because it's also one of the few moments that we see Anthy's true emotions. The Wakaba fight is awesome because it's so intense for all three of them.
