My grandmother got dramatic fever when she was pregnant with my uncle eight years after giving birth to my mom. Because she had her tonsils removed she had a strep infection that went unnoticed until it turned into Rheumatic fever. She has two leaky heart valves because of it.

When patients started having RF symptoms, only 25% will have positive strep throat culture. Doesn’t this mean when they develop RF, most patients are clean of the group A strep infection? Then, why do we still treat RF with long term penicillin?

How do we diagnose rheumatic fever when titre testing is done too late After strep throat infection whilst being over looked by a skin infection and put on doxycycline for 6 weeks ? Could the titres be low after doxycycline and onset of rheumatic fever ? patient has subcutaneous nodules, arthritic movement , echo findings of mild tricuspid valve regurgitation , traces of mitral and aortic regurgitation ? Ecg , crp, esr all normal . Are there any other tests to prove it ? M protein testing ? Anti-hyalurinidase ?? It’s a diagnostic nightmare without those asot-adnb coming high . Auto-immune /Lupus , rheumatic/septic arthritis all clear . Need your help medicosis perfectionalis 🙏

I am currently having pharyngitis, and having joint pains typically on my knees and my phalanges…my doctor only indicated from my chest X-ray a 8mm nodule on my right lung lower zone. What are the chances that my dr might have missed out of rheumatic fever? Also is it even possible for me to not shown any JONES symptoms? Or perhaps I am not sure of the specificity of my pain? Been coughing for coming close to 2 weeks now and my dull aching joints especially at my fingers bothers me… sigh idk what to do now…
