An oculoplastic surgeon is especially important for treating drooping eyelids, as general cosmetic and plastic surgeons are not trained to address the levator muscle that lifts/opens the upper eyelid. Often in these cases, only skin removal is performed, which can lead to complications associated with a skin shortage.

I have wanted upper eyelid surgery for some years but could never afford it but now I'm in a situation that my Optometrist has asked my GP to refer me for Ptosis surgery which hopefully will be done on the NHS. Hi Shirley Cranswick, Eyelid ptosis is considered a health issue, not just a cosmetic issue, since it can affect vision, so your national insurance should cover the surgery. However, keep in mind that the surgeon may focus on eyelid function only, and not appearance. Hi Julia Lee, While I do know some doctors from around the country, we all have different practice focuses and specialties. I suggest you look at the before and after photos of prospective doctors, ask them if they can differentiate eyelid ptosis (eyelid muscle issue) from dermatochalasis (extra eyelid skin), and choose a doctor with whom you are most comfortable. Hi @lindawitt4661, Thank you for watching. I do understand your hesitation. As explained in the video, risks are reduced by the expertise, and experience of the performing surgeon. Complications are most often due to surgeons performing surgeries outside of their expertise, and/or are not yet experienced in a particular surgery. The difference between successful cosmetic eyelid surgery, and complications are mere millimeters, so planning, expertise, and experience are all needed for a successful, and safe outcome. Thank you for this information! I was wondering, can blepharoplasty be done on just one eye? I have some pretty asymetrical eyelids, and only the right one is uncomfortable, so it would be fine with me just to leave the left one as is.

Hi C L, It depends on what is determined in a physical examination. Eyelid height asymmetry of 1-2mm between the two eyelids is within a normal range that is unnoticeable by others, so I wouldn't recommend surgery for slight asymmetry. If it more than that, an examination is needed to determine if it's caused by something other than bone structure. I hope this clarifies things for you.

Dr Prasad I found your video very intersting,I just had eye lid surgery and one eye looks not right either too much skin taken off or the shape of the cut is not the same as the other or both? Hi michelle wright, Sorry to hear about your problem. At 6 weeks post-op, you cannot determine yet if it's the final result due to temporary swelling and ongoing healing. You need to be patient and follow up with your surgeon as healing continues. General plastic and cosmetic surgeons are not trained to perform eyelid ptosis surgery, so you will need to see an oculoplastic/oculofacial surgeon like myself for a proper diagnosis. I had upper/lower bleph just over a month ago. I thought the tightness would go away by now, but it's still very tight & I can feel the tug/pull of the incision each time I blink. I hope it will go away. My eyelids are also still numb. And, I can just barely close my eyes but there's no extra give so I can't scrunch my eyes closed (like I used to in the shower to keep water out). I guess that's the "new" normal. Just wish this tight feeling around the incision would go away. I heard it goes away in 2 weeks but like I said it's been just over a month.

Hi @Oivey2000, While minimizing tension on the incision is important to prevent visible scars, it could be a nerve issue since you still feel numbness Your surgeon can help guide you in the healing process. Dear Dr Prasad, I underwent a transconjunctival blepharoplasty three and half weeks ago but I am very depressed and unsatisfied with the outcome. I am now left with hollow tear troughs even though the eyebags are gone. They make me look tired and older. Hi Chin Kaar Ti, Sorry to hear about your problem. There is an art in lower eyelid surgery in leaving a little fat behind to prevent a hollow appearance. Injectable fillers may help, but I would need to see in an examination where the hollowness is before I confirm where I would place them. There is an art to filler placement as well for best results. Dr. Prasad, i have MGJWS with strong winking and ptosis about 3 mm in resting position. Looking at the mirror, i suspect that ihave above the ptotic eye also exessive fat(dermatochalasis?). Hi kadi diwup, If you have eyelid ptosis, fat removal alone, and even eyelid skin removal, will not be enough to lift your eyelid by 2-3mm. People with ptosis who only have upper blepharoplasty with the skin and fat removed only will still have drooping. If too much eyelid skin is removed, you will likely not be able to close your eyes fully. If too much fat is removed, your eyelid and brow will look hollow. Operating on the levator muscle or Mueller's muscle are the only ways to properly raise resting eyelid height. I hope this clarifies things for you.

The frontalis sling bypasses the levator muscle, and uses the frontalis muscle of the forehead to lift the eyelid. I am a 32 year old woman who had a horribly botched blepharoplasty. My upper lids were cut way too short and my lower lids were retracted. I had successful lower lid retraction surgery to bring my lower lids to the right position. Now, my eyes are both able to close, but they still both come open at night from severe skin shortage on the upper lids. I strongly feel I need skin grafts on the upper lids to give myself enough tissue. When I look downward, skin only covers half the eyeballs, unlike most people whose eyes are all covered with skin when they look downward. It seems obvious to me that I have a significant skin shortage but some local doctors look at me and after measuring the distance between my eyebrow to my eye say I have enough skin even though my eyes still open considerably at night and continue to let me suffer. Why do they say this? Is it possible to get a good cosmetic result from upper eyelid skin grafts when all the healing is done? I heard about lasers and PRP being done to help with the healing.

Hi Kanika Sharma, Upper blepharoplasty is the removal of redundant skin and fat, so it's not the right surgery for ptosis since it is caused by a stretched, detached, weakened, or defective muscle that lifts the eyelid. General plastic and cosmetic surgeons who perform upper blepharoplasty are not trained to treat eyelid ptosis. Eyelid ptosis can recur as the levator muscle that lifts the eyelid is engaged thousands of times a day to blink, so this amount of activity over years can loosen, stretch, or detach the levator muscle that was previously treated with surgery. A proper ptosis examination is needed to diagnose for muscle function, and what type of treatment would be appropriate for you. I suggest you take some time to get used to it, and follow up with your doctor if there are issues like the eyelid looking too low, or raised too high.
