I really enjoyed this stream when it happened live! I've been running weird Heretic challenge runs recently, like spawning as Heretic and removing the Heresy items, and I can confirm that it's possible to beat the game like that. It's more fun if you don't allow yourself Lunar items, since otherwise you can take Transcendence to nullify the negative health regen, or just grab Heresy items. Another fun run is to spawn as Heretic and give yourself Egocentrism, which pretty much plays out like this run right here.
