Yes, it's like he tricked the die to just keep on rotating and before it realised it was biting metal it was already halfway there ... *joke* but that is what it looked like.

In the states we would call that a shoulder bolt. As an example, the hollow bolts that hold brake caliper hoses are called banjo bolts.

I know what you're talking about, we call those banjo bolts as well. The name is a bit confusing for me, as it is also a musical instrument :)

Next time you have a high strength bolt to turn down detemper it first, by heating to bright cherry red and allowing to cool slowly. Gets the stress and work hardened area out from the rolled thread area, making machining easier. Simple afterwards to harden it again by heating to bright cherry and dropping in old oil, then heating to dull red and allowing to cool slowly to temper it again. Bonus is the oil quench leaves a black oxide layer on, which accepts oil well and is relatively hard wearing.

I think the problem was a bad insert that wasn't shimmed properly.

After hardening it, heating it to red causes it to anneal. You only need a spring temper on it, about 500°f/300°c. Carbide will cut hardened steel no problem, it's fine.

Great example of using the metal lathe to fix other machines. Those hardened bolts can be really tough on carbide inserts.
