In a one year time jump, she is seen with Father Gabriel, playing and running around and in a deleted scene of another ten years, she is with Judith, R.G.J, Gracie, Mariam, and possibly with Adam, Ezra, Aliyah, and Rosie. Joshua was taken with her mother to a safe place, sent by Negan, and Hershel was kidnapped by The Croat. Lydia remained at Alexandria, Hilltop, Commonwealth or Oceanside, yes it's also revealed in a deleted scene that Oceanside is Alive.

Even though her death wasn't originally planned, it still gives some form of closure for the show. This is basically the last main character death of the show, and it is peaceful. It's showing us that brutality is a thing of the past (previous deaths of main characters were either uncomfortable, painful, violent or even all together). Her last moments aren't just her suffering, it's her with her family and in a comfy bed.
