It's always about art style, not graphical quality, and this game seems to do a masterful job with the art while keeping the graphical quality itself at PSX level, also doing a really good job with some of the other bits that actually successfully style it that way (I feel like the way they handle the water splashes as sprites is a really good example of this)

Step 1: learn how to make maps with ultimate doom builder Step 2: learn how to make and implement custom sprites, textures, and sounds. Step 3: learn how to add custom enemies and weapons. Step 4: learn how to modify game behaviour. You now have all the tools necessary to begin making something like this.

For maps, use Doombuilder 2 For tinkering with things beyond just map-making in WAD / PK3 files, use Slade. For graphics and sounds you'll need to use other programs to make these and import them into your wad. IWADs (base game files, doom.wad or doom2.wad for mods) you have to obtain on your own if you're just modding base Doom/Doom2, but in this game's case the creator made their own IWAD because it's a total conversion meant to run with GZDoom, which is source port of the doom engine.
