I feel #3 happens to me but when I try to throw flat/not hyzer. Would that be possible?

Sure. when you throw hyzer hopefully everything is tilted on hyzer: hips and shoulders and arm. When you throw flat you may have everything flat(er) which is not great mechanically... you can still throw flat if you want to, but dont expect your brace to be as strong or as able to contain momentum. When people throw flat there generally tends to be less space for the disc, which kinda creates problems like rounding... try to find a way to keep the space you have on a hyzer when you do a flat throw...
