Plane of Fire? Pandemonium or the Abyss, hands down. I'd even accept the Beastlands considering how many things try to kill humans there. There are things there that should not exist in such numbers, nor with such... cunning. :)

So our DM/Campaign has been dealing with a plague cult in the early sessions and they've been quite a few references to vermin enemies. So I'm totally expecting this to get thrown at us in the future.

Trickier Hades good! The whole reason I started this series was to try and help DMs bring back that sense of awe and wonder when their players encounter something new.

If you want to be extra nasty, give this thing the ability to have a part of it enter a deceased body (like the one of a downed PC for example) forcing this body to make a CON Save. On a failed save another ruin demon bursts forth from the corpse 1d4 rounds later, bearing another face - that of the deceased character. Not only that, but the character can't be resurrected with anything lower than Resurrection, and even that only after the second Ruin Demon has been completely destroyed, because it holds the soul of the devoured creature similar to the way a Magic Jar would.

I think using a spell or item that induces nausea/vomiting could be a counter to the infestation. It’s thematically gross/awful, and it rewards cleverness in players while also forcing them to deal with the side-effects; being sick is better than dominated, but it still sucks.

The first thing that came to mind when hearing about these things was the plagues so I think they could fit an egyptian setting really well maybe the party is investigating some grave robbers going into a tomb when they come across those grave robbers fighting one of their own and perhaps they even kill the infected on, but upon seeing the party blocking the only exit and in fear they run deeper in and just as the party passes the corpse these flies burst out of his fresh meat!

You could even make it a swarm of scarabs instead of flies! That would fit an Egyptian setting even better.

Party is watching a Cadaver Collector doing its thang. Cadaver Collector picks up a bloated corpse and as it punctures the corpse on their back, the Ruin Demon emerges and attacks the CC. It gets under its armor and starts messing with the machinery in the CC. That should really highlight how awful this thing will be

Not many people know this but Beelzebub means "Lord of Flies" so you can totally change the stats to a demon Lord and make it a BBEG.

Something like this could have a Hideous Drone, as the flies wings create a horrid buzz that interferes with Concentration, granted characters that are disadvantage on their checks.

Reminds me of the horror movie. Don’t remember the name though. But swarms of flies entered human bodies where it was possible (eyes, ears, nose, mouth, open wounds,…), then laid eggs, took over the body ( I thing by eating the brain) and then proceeded to attack their former friends. In the end they were all controlled by one guy.

For some reason I feel like seeing a player dragged a way by a a hoard of flies now to be eat a live . May be a the A hole villager that throw dung at the party will do Some how the I fell like I can work a hint in. Only to have to have them cover in some thing stupid that the ruin demon will not eat. Mud , jam or dung LOL.. Only to get them some were I need them May be something Like the gypsy from Leonard part 6 that just give them a jar of magic jam . That cover them when opens lol
