In this video, Dr. Praveen Kammar (Consultant Surgical Oncologist, SSO Hospital, Mumbai) will tell us about the staging of stomach cancer & how stage 2 and stage 3 stomach cancer is treated.
00:40 How is the staging of stomach cancer done?
Staging is the process of finding out how much cancer is in a person's body and where it's located. It's how the doctor determines the stage of a person's cancer. The factors that decide the stage of stomach cancer are as a percentage of area affected by the walls of the stomach, damage done to the lymph nodes & area affected other than the lymph nodes.
1:29 Layers of stomach
The stomach is much like a bag with a lining. The stomach is made of these layers:
The inner lining of the stomach is called the mucosa. The lamina propria is the thin layer that consists of connective tissues. The submucosa is the second layer that supports the mucosa. It is rich in blood vessels, lymphatic vessels, and nerves. The serosa is the last and outermost layer. It’s the lining that wraps around the stomach to confine it.
1:53 Stage 2 stomach cancer or stage 3 stomach cancer
Stage 2 or stage 3 stomach cancer is involved beyond the muscle layer and has also affected the lymph nodes. An expert advises a patient to undergo endoscopy, biopsy, and also CT scan & PET Scan. If the stomach cancer is in the mucosa, it can be treated by endoscopy itself.
Treating stomach cancer with endoscopy must be done by a specialist and an experienced doctor only.
2:40 Treatment for Stage 2 stomach cancer / Treatment for Stage 3 stomach cancer
The treatment of stomach cancer varies according to the location of cancer, its spread, and various other factors. Stage 2 or stage 3 stomach cancer is often treated with chemotherapy so that the cancer cells do not affect other layers of the stomach. Giving chemotherapy prior to surgery gives good results.
Laparoscopy in combination with laparoscopic ultrasonography is a new staging procedure for tumors of the abdomen. This is important to be done because cancer cells less than 5 cm are not seen in Ct scan or PET scan.
5:26 Surgery options for Stage 2 or stage 3 stomach cancer
This depends on which part of the stomach is affected by cancer. Doctors advise either subtotal gastrectomy or total gastrectomy.
