If they are used among really close friends, its most likely jokes, and its like any nicknames, no hard feelings. However I never heard husbands and wives called each other like this. In case this word is used seriously to describe someone, that means the person always does shameful or disgraceful things to hurt or harm other people, without showing himself/herself. Like hidden behind the scene to watch others to fall into the trap he/she made. There is NOTHING to do with addiction. Some Chinese words don’t have the same meaning if you see them as separate words versus when they are combined. But this is indeed a bad word, avoid using it in any circumstances.
