Came across this as I started to look into "Safety Third", which also mentions complacency as a root cause. I had summer jobs in a chemical plant (as a pupil/student) and they were very big on "Safety First". I saw two flavours of that: Know your trade and do not forget to take safety into account. Or "It''s all Safety First here, somebody has taken care of it, I don't need to ..." (When they introduced a program named "Rhythm - Remember how you treat hazardous materials" it was obvious that something had blown up in some other factory)

Maybe took a bit too long to get to the point but I think it was a good one. Just a bit silly tripping over the pencil and the cartoonish scream. I thought they were going to show the factory guy spilling something on himself and the factory exploding but I’m not sure that would be much more effective anyway.
