The affidavit should be in your name, stating your relationship with the person (your father) having the name issue. Coz its your visa application and all the supporting documents should have your name mentioned on them.

Well in that case, you should try to get it corrected on your marksheets first and then apply for passport. OR get the passport as per your marksheet (incorrect name) and make an affidavit later showing both are the names of same person only. First process is a better option but will take a lot of time. Second one is quick but might cause some problems later.

Dear Sir, I Also Have This Kinda Surname/Last Name Problem:- In All The My Documents (Educational, POI, And Even In My Passport) I Have Surname/Last name as "PARMAR" but In My Parents Documents They Have "DARJI" as Surname/Last name in Their Documents. Better to attach this affidavit with your visa filing then. Vaise adhaar and pan ke name ka itna koi issue nahi hota.

You should justify the missing surname by attaching this doc. But its not like that attaching this doc will get you the visa. It depends on your profile, course, sop and presentation.

If all the documents have the same name as the first name, how come the passport was issued with a different name? Anyways, For a permanent solution, get the name corrected on passport itself. If not, this affidavit is the only decent option left.

I have single name in my educational Documents till graduation and after marriage their is addition of my spouse surname in my ID card as surname was important to me mentioned for passport so now there is first name with surname in ID card and passport but no surname in educational documents and I have to apply for Canada visa what to do now… should add surname to educational documents or no need of it.. or an affidavit will full fill the needs please let me know. No need to get it changed on your educational documents, as it can be a long and hectic process. Just attach this Affidavit with your application. Make sure to use the same format as shown and mention both the names(with and without surname) in your affidavit.
