I always interpreting the ending of The Piano Teacher as her taking control again. She's in control for essentially the whole movie, but then the rape scene marks the point in the film where she finally truly, is not in control anymore. He assumed he had her in his control so he acted like nothing happen and assumed she would do the same. Her stabbing herself and leaving is her rebelling against his control.

I recently saw the film in theaters with the motto: "Mit Freud ins Kino!", Freud meaning Sigmund Freund. After the movie has ended, we all stayed in our seats and discussed the movie as a crowd. Amazing experience for one very unique movie.
