It really depends on what choices you've made back in episode 4. If you took Radar with you back to Beacon Town, Romeo will sacrifice himself, regardless if you decide to leave him behind or not. However, if you save Fred's People, that's when you can save Romeo, because of the Giant Enderman fighting and distracting the glitchy Prismarine Colossus.
