Uterus की अंदर की lining जो हर पीरियड के दौरान , बहार निकलती है उसे Endometrium या uterus lining कहा जाता है। Endometrium , uterine cavity में स्थित होता है। पर काफी बार यह endometrium uterus के अंदर बढ़ने के बजाय uterus के बहार बढ़ने लगता है जैसे की ovary में या uterus के पीछे बढ़ने लगता है तो उसे endometriosis कहा जाता है। Scar Endometriosis यह एक प्रकार का endometriosis है जो चोट (scar ) पे बढ़ता है। ऐसी स्थिति में जब भी हम Scar के बारेमें सोचते है तो पहले महिलाओ को C-section या cesarean का scar दिमाग में आता है। जब endometriosis या uterus की lining इस cesarean के scar के पास बढ़ने लगता है।
Scar Endometriosis Symptoms & Diagnosis
यह endometrial lining scar के पास बढ़ जाती है उसे थोड़ा रक्तस्त्राव होने लगता है जिसके कारन Scar के आजुबाजु में बोहोत ज्यादा दर्द होने लगता है और यह दर्द खासकरके period के वक्त होता है (abdominal pain years after c section)।Scar endomtriosis का निदान करने के लिए small part ultrasound किया जाता है जिससे doctors को उसका size और affected area पता चलती है।
Scar Endometriosis treatment
Scar Endometriosis treatment यह उसके अकार पे निर्भर (size dependent treatment ) होती है। अगर वह endometriosis, scar के नज़दीक है और 1 cm छोटा है साइज में तो शायद इसका दवाइयों से इलाज हो सकता है। यह दवाई गोलियों के रूप में दी जाती है। यह दवाई ६ महीनो तक दी जाती है। इस दवाई का नाम है Dienogest | यह एक ऐसे दवाई है जो endometrium पे काम करती है और उसे shrink कर देती है। पर यह permanent solution नहीं है।
अगर Scar Endometriosis का size 1 cm से ज्यादा हो तो काफी बार डॉक्टर्स dienogest देके वह size कम करके फिर surgery करते है। इस सर्जरी में Scar Endometriosis के साथ साथ इसके बाजू बाजू के १ १/२ cm का भाग भी निकलना पड़ता है जिसको Wide Local Excision (WLE ) कहा जाता है। यह करना बोहोत ही महत्वपूर्ण है क्योकि इससे Scar Endometriosis दोबारा होने के सम्भावनाये कम होती है ।
Scar Endometriosis Surgery
काफी बार Scar Endometriosis पे additional surgeries भी की जाती है जब डॉक्टर्स को लगता है की यह काफी अंदर तक फैला हुआ है। Scar Endometriosis में सर्जरी के अलावा कोई permanent solution नहीं है। अगर आपको डॉक्टर ने दवाई लेने की या dienogest लेने की सलाह दी हो तो वह आपको ६-९ महीनो तक लेना है और इसके बाद ही आपको इसका result मिलेगा।
Scar Endometriosis की सर्जरी काफी technically challenging सर्जरी होती है क्योकि Scar Endometriosis के साथ ही २ cm का बाजु बाजू हो हिस्सा भी निकलना पड़ता है और सर्जरी के बाद यह ध्यान में रखना पड़ता है की यह वापस फिर कभी ना आये ।
Scar Endometriosis जितना ज्यादा फैला हुआ है उसपर से recovery तय होता है। काफी पेशेंट्स को physiotherapy की भी जरुरत पड़ सकती है।
