A little feedback: The first thing to check when trying to diagnose septic shock isn't a bunch of lab tests, but rather an examination of the patient (which goes beyond just a check of the vitals). Blood cultures are not used to diagnose septic shock, they are used to identify its etiology. The foundation of shock treatment is prompt restoration of normal hemodynamics; in other words, fluids and vasopressors are more important and time critical than antibiotics. Using CRP and ESR to monitor "progress" in the treatment of septic shock is simply not done in real life. These tests are meaningless in this situation. The best way to monitor progress is to serially examine the patient (tracking vitals, monitoring CVP, following mental status, etc...), monitor urine output, and follow Cr and lactate.

+Eric Strong Bravo for your comment... I agree! I recently was taken by Emergency Squad to a hospital w/left flank pain... I was taken back for a CT scan which showed a kidney stone with "infection or rupture".... I was given pain meds and sent home to pass the stone. The next morning my family took me back to a different hospital and I was already in septic shock... my kidneys and liver had shut down, my blood pressure had plummeted.. but yet the 1st hospital in spite of seeing on the scan results there was infection, gave me pain meds and nausea medication, no antibiotics by IV which is what was really needed! ! I'm glad the 2nd hospital knew what to look for and took action so I could type this comment today....I'm a survivor!

prior to being taken to the ER the next morning after having already been at the 1st hospital, I remember moaning all night and the next morning uncontrollable shaking and a fever....got the ER and my kidneys had already shut down, blood pressure was 67/32 so very low...liver had shut down so I was probably jaundiced as well....

let's say the symptoms have been controlled and the pt is hospitalized for a few more days. Do we still use CRP and ESR to check the inflammatory status then? :)

ESR/CrP are acute phase reactants and not really useful in Sepsis or later.

A truly frightening experience. If you get disorientated, confused with infection get to hospital urgently

I agree......for test purposes, unfortunately, this is what they expect you to know. We all know the test world is unrealistic, ironically. However, thank you Khan Academy for your visual explanation....extremely helpful.

My brother died 15 days ago unfortunately doctors did nothing like this to save him he walked in to the hospital with pneumonia one week later all his organs shut down.. neglected nurses and doctors now he is dead!

If septic shock is about a massive dilation of the vessel, why would we give fluids that will even dilate the blood vessel? Wouldn’t that cause more hypotension?

The fluids they give them is albumin. Thats not “refilling the blood”. Its like vitamin blood without it being blood. It has vitamins that is lost with sepsis. There is a protein in it that “ holds the fluids” .

Hello, I've been diagnosed with folliculitis since 2013 caused by staphylococcus infection. I also had furnacles & fear that I may have Sepsis. I heard that this is a uncurable deadly disease & I'm afraid to die from this. I don't know what to do.

I think I may have experienced septic shock and for sure anaphylaxis shock after the yellow jacket bite. ER doctor didn't mention the septic shock but am scheduled for colon screening. 😫

Metabolic Theory of Septic Shock Please do a search for the above Core tip: For decades septic shock has been attributed to an over-active immune response. However, immune modulation has failed to reduce mortality, casting doubt on a direct causal role for the immune response in the development of septic shock. A closer look suggests that septic shock is the result of a generalized build-up of hydrogen peroxide, a toxic cellular by-product generated as a consequence of the hypermetabolic state that accompanies a systemic immune response. This finding points to the systemic accumulation of hydrogen peroxide as a significant risk factor for the development of septic and non-septic shock syndromes.
