I faced the same issue bro, but now it's been reactivated, but what we have to do in order to get this form is to visit SCE and ask the reason and ask what we should do in order to reactivate the suspended account, and they'll send an email attached with this appeal form which we have to fill and send this form and reply in the same email. Note: - Pay by visiting the bank on your Iqama, the reason for SCE membership activation, in this way they ll receive it immediately, or else it will take a week to reach them if you pay online.

You have to visit sce office and inform them then they will immediately convert your membership temporary to permanent..
