The pharmacy tech whom I'd never before seen sold me the wrong size points for my two monthly doses for male (cis) testosterone replacement therapy which I've been using for years. In fact I'd been neglecting my therapy and when a couple of weeks ago I did some labs my Dr. told me that I was very low. So I've made a renewed effort to be more compliant. Anyway the shots hurt bad and today I actually drew blood. I chose to not return to the pharmacy and get new equipment because frankly I'm embarrassed by the treatment I received from the young tech who was unfriendly and referred to me, an older gentleman and customer as "Bud". I thought he resented my prescription and myself. Anyway. I successfully did my injection but for the first time today I was afraid that I was doing something wrong. The problem was the gigantic points this guy sold me though so next month I'll be more attentive to the selection of points I'm offered. You have a great screen presence and I sense empathy so I'm commenting here instead of another health care professional with a very good video on the topic. Thanks.

I used a 1.5 inch needle today for my testosterone shot, usually use a 1 inch. I am a smaller guy, not a lot of fat back there.. But right after I did the injection, my lips on my face started burning bad, and my right toe ached a bit.. Something was not right? Does anyone know what could have caused this? Is stopped after about 40 seconds. I seem to be okay now.. Just wondering what could have cased this?

Those trt shots are going to hurt regardless. Ive been on trt for 2 years now and 7/10 times it hurts some times worse than others. Keeping the muscle relaxed is huge if he tenses up guaranteed its going to hurt. Not so much while doing it but for 3-5 days after its going to be sore.

These next four words I say will make life much easier for you. I had to thank a friend who shared it with me..... WARM UP YOUR TEST!!!! I take a bowl and fill it up with roughly 1/2 inch to 1 inch of water. I heat it up in my microwave for 52 seconds on HIGH. Every microwave may need different times, but that is the sweet spot for me. I only have the bowl and water go in the microwave and not the test. Afterwards I take it out and sit the bowl on the counter. Then I pick up my container of test, and sit it down in the heated water. I let it sit roughly 5-10 minutes. Then I use my alcohol swabs to wipe the top of the container before drawing my dose, and also wipe down my upper cheek or leg for the injection. I find warming your test minimizes post injection pain or PIP which sometimes can hurt. I hope this helps someone.

Alim, and medium build people do not need a 1.5 inch needle buried all the way in. They just don't have that much meet there. Also insist on a 25 gauge needle. Nurses will default to what is easiest for them m, and that us a larger bite needle who g hurts the patient, and causes scar tissue. In someone that needs weekly, it daily injections you will quickly realize that you are doing damage with the 21 gauge needles that doctors will prescribe, or nurses will default to for their own convenience. Everybody's anatomy is different so be your own advocate. It's your body, and many nurses are just trying to get through the day as quickly as possible so thet can get caught up on paperwork. Speak up, and ask for these things. Insist on a 1 inch needle, and 25 gauge needle fir your own comfort and to minimize scar tissue, and the possibility of hitting a nerve. The sciatic nerve can be reached on a thin person with a 1.5 inch needle th at is pressed all the wat in. Bone can be hit as well. She was pretty far back and a little high in the gluteus nedius in this video, which would define call for a shorter needle in this patient. My uncle has permanent nerve damage from getting jabbed in the sciatic nerve during a hospital stay. When you are unconscious and no one is there to watch out for you, you just have to hope you have a nurse that gives a damn about the patient .

When we aspirate blood in the syringe, our concern is that the tip of the needle is in a larger artery or vein. That way it wont be intramuscular but intra-arterial or intravenous injection. Deep Intramuscular injection in the front of the thigh is not safe by a lay person either.. There are branches of femoral artery, vein and nerve, The outer middle third of the side of thigh ( muscle Vastus Lateralis) is safer. There is are no major artery,nerve or vein in that area Harish Malhotra MD

You're missing the absolute best place for intramuscular injections- the ventrogluteal site. The ventrogluteal site has less subcutaneous tissue and a thicker muscle mass than the dorsogluteal site. Therefore, the needle has a better chance of reaching the muscular tissue in the ventrogluteal area than in the dorsogluteal area. In various studies it was found that most injections given to the dorsogluteal site delivered medication into the fatty tissue. Pain receptors are found within the subcutaneous layer, not in muscle tissues and so injections administered into subcutaneous tissue may be more painful. I feel zero pip with ventrogluteal site. I mean zero.

I've been injecting myself in the same spot for over 30 years. A doctor once told me there is a cavity in this area that's ideal for this very thick drug. when he left all the other doctors who gave me injections made me limp in pain a day later, so that's why I have been doing it solo!

I’ve been a hospital nurse for 16 years and we were taught this method and I follow it. But…after thinking about it….what is the harm in blowing an intramuscular capillary on IM injection? The force of the injection would blow the capillary apart and the med would be absorbed into the muscle just fine. Perhaps there’d be more bruising, but this happens on subcutaneous injections also. Sometimes we hit capillaries inside fatty tissue. But muscular tissue is more vascular anyways, so it’s not like the effects of hitting a capillary would be that dramatic? Hitting a large vein and getting heavy blood return could be more of an issue, but a capillary? Also, many of the meds we give IM are also indicated for IV use. I just want to know if my logic makes any sense or if someone can tell me why hitting a capillary would warrant a new injection site. Not trying to be a PIA. 😄

The injection must be done on the superior AND external cadrant (as shown), but the MAJOR reason has nothing to do with pain but with the risk of hitting sciatic nerve OR the superior gluteal artery and so on.... You should avoid to do such things by yourself.

Your English is better than most Americans. I get a few drops of blood too sometimes, but it is not uncommon and doesn't affect the shot. When you inject, try to go straight in without jiggling the needle too much (up, down, sideways and if you take your had off of it, it will fall just due to gravity). Actually put your thumb on the syringe before you inject so that it is not 2 movements. I mean inserting the needle, one movement, then finding the pusher, another movement. The "jiggling" of the needle is what causes the blood for me. Listen to what she says, and inject where she says. Yes, I know that this is not fun, but the less you think about it the better or you could think that you are not doing it to yourself, but to somebody else. Hope this helps. Good luck from Florida!

Best tutorial on the ventroglute. As many..shots as I’ve done in my life you’d think testosterone in the hip wouldn’t be nothing. I’d never think you could tap a vein in the glute though. I’ve watched red come back in a syringe a lot in my life, not anymore but never when it’s straight into a muscle so that’s kinda wild to hear that it can still happen unintentionally.

For new doctors, nurses and students on board, massaging the area after an IM injection is not a recommendation anymore bcz you might dislocate the medicine into the subcutaneous tissue instead of the muscle, plus pulling to check if there is blood is questionable at the moment, studies say it's not of many benefits in comparison to the disadvantages; usually, those areas don't hold big blood vessels and you would have to change the needle and put the patient under another injection. please update me if I'm wrong.
