Thanks for explaining the science behind this. The fibroblast forming a latticework make absolute sense. I never wanted to have an ablative laser until understanding how this therapy can truly work.

I've done both, with great results. 1999 was the laser and around 2015 the needling.

Lasers facial resurfacing results in common complications, they are, raised lines that come and go varying with temperature changes, dermal thickening, scarring and there is little that can be done when these complications occur. A relative suffered all of these and was informed by the plastic surgeon who carried out the procedure he didn't have a magic wand to wave over her. She was left devastated and the only laser expert that she sought help from who was willing to help her, was a laser expert in Belgium, who stated that due to the common complications that she had and he was aware of, his practice no longer carries out laser resurfacing instead he uses the Tel Aviv chemical peel face mask as it has shown to have fewer complications and has a better outcome. His practice had found that mega doses of vitamin B complex lessens the lines and improved the overall damage done by the laser procedure. Unfortunately doctors don't always mention complications of the procedures they recommend. Best wishes from Europe ❤

Really well, thanks! My doctor use a somewhat low setting all over n a stronger setting on a few spots so I healed pretty fast. I did breakout in a rash due to the aquaphor after a few days of using it so I switched to the moisturizers I usually use which my doctor said was ok. I’ve noticed the pores on my nose r smaller, the fine lines around my eyes have diminished, n a few of my dark spots were already lighter.

Had full face TRL laser 2 months ago, this is not for the faint of heart. Similar to Co2, this is a serious procedure (2 hours + of probably 100 painkilling injections plus demerol, ativan) and brutal 1st week caring for a full face oozing wound. Week 2-3, worst is over over but crazy flaky skin so although I could use concealer, it turned into oatmeal haha. Fast forward to now, i'm still cautious about exertion, minor roughness, bent over positions which all cause pin prick blood spots, redness, delayed healing but wow, amazing results! Glad i did it, i honestly can't remember when my skin looked so smooth, taught, spot & wrinkle free but from here on, plan to maintain with more frequent/less downtime procedures.

Oh my goodness, what you went through! I'm so happy that you feel it was worth it. May I ask what age your skin was considered when you did this? Odd question in some ways, but one's chronological age is not always synchronous with one's skin age (sun damage, smoking, etc). i.e. I'm 55, skin age 48 (thanks to a lot of IPL treatments in my late 30s/early 40s. I'm wondering because you say your results were excellent (which is AWESOME to hear, since you went through quite a lot of suffering for a few weeks, it seems). Thanks in advance, and no worries if you'd rather not say. : )

@@le_th_ I'm 58, healthy and otherwise age defying but my face was looking tired. I'd done a few laser treatments & botox over the years but nothing aggressive. It was time for an overhaul & time to commit to regular maintenance procedures at a great comprehensive clinic.

First of all, thank you for this valuable information. Regarding microneedling, I would like you to explain to the viewers the difference between the products sold for home use and what we have in clinics, and that to benefit from the process to stimulate collagen, the length of the needles must be more than 1.5 ml and it varies according to the thickness of the area (I think). It is something that many people ignore, and we will not find someone better than you to explain this to us and the public. Thank you

Great video, Doc! I have a Philips Reaura laser which is no longer available. I’ve found it to be very effective over the years. It’s to be used twice a week on each area for 8 weeks. Question: If I use it like this, can I use Retin A in the between nights? Or, can I use the Reaura every night?

I just finished using accutane Used differen the night after, Woke up in the morning. Applied checmichal sunscreen and my skin was hyper reacting to the sunscreen and I got box scars from it. Can u imagine? This happened last week. And now I am trying to choose what kind of procedure should I get to treat the box scarring that happened on my cheeks. But watching ur video made me realize that I should steer clear from all procedures and acids, for a year at least to restore my skin’s strength back first. Thank you for the warning at the end of your video!!! I am grateful, you helped prevent a big accident 😢 thank you sir

Ablative lasers may be used for example over grotesque scars that are large (reconstructive)..vs non ablative a flat scar where you'd like to improve the esthetics of the scar to make it appear like normal skin.

I had the IPL lazer 15 years ago and loved the results. Just now needing it again. No age spots still, (I do peels bi-monthly), but am starting to see very little fine-lines around my outer part of my mouth where the marionette lines end. At 62, not bad. I think I might try the micro needling on them.. if it does not help I will go with the lazer. Thanks Dr.

Very informative video! I know this is an older video, but can you please tell me what type of laser the Tetra CoolPeel is? Ablative or Non-ablative? I've recently had 2 treatments but haven't noticed any improvements. I'm wondering if there are things going on behind the scenes that will improve my skin (like collagen building) that I will notice months from now. I'm not sure if i should get another 1 or 2 more treatments. Thank you.

Microneedling with radio frequency can cause fat loss. Be very careful with this treatment. Personally I’d prefer microneedling without RF.

Depending of Genetics & Skin Types-Subcision created loss of Adipose for me, then 3 Laser Treatments gradually increased Collagen and Adipose Tissue.

I'm looking into getting laser treatment done for my severe acne scars and I was leaning towards ablative but not after learning of it's potential of fat loss.

Hi. I am a 62 year old fair skinned female with very old burn scars on the right side of my face. I was burned when I was 4 years old. My skin on the normal side of my face is quite good with very little wrinkling, except for fine lines over my lips. I have booked an apointment to do microneedling with radio frequency on the 18th of april. Is this something that you would recommend to improve my burn scars. The picture that you see of me on my google account is a bit airbrushed. I would appreciate your input.

This is the first time I fully understand all the types of lasers! I noticed an accomplished Doctor used laser over the whole face except he used Ultrasound on both sides of the jaw; subsequently I read that ultrasound can remove fat so one might want to use it at the sort of hanging area at the sides of the chin. Could you elaborate?

What is your opinion of utilizing a micro needling pen at home? Will it help topical treatments penetrate the skin OR a waste of time and money?

Thank you for the explanation. I’d like to know which would be best for taking down small bumps in the skin to smooth it out? I know the microneedling does well to improve indents but I’m more interesting in taking down some bumps. Thanks!

Radio frequency micro needling needs to avoid depths significantly beyond 2 mm in most parts to avoid burning fat. It should also be used with topical exosomes (avoid injections for safety) and all quality exosomes must be scrubbed for diseases AND be added during the micro needling session and up to 3 hours after it since pores are still open (and sensitive to germs as well so hygiene is a must). At least 3 sessions a month apart each will be required for very major improvements (it takes over 5 months to get final results). Diet, rest, chemical peels (3-4 sessions), and considering hydrolyzed collagen (all peptides ideally) and hyaluronic acid at safe and conservative doses will also build up collagen during periods of micro needling for at least 100 days.

I just took Morpheus 8 + Fraxel Dual + PRP Treatment and am really amazed with the result. I feel my face is tightened much and can even see the change on the mirror. I took several other treatments similar to it before (one of them was also micro-needling with RF just as Morpheus 8) , but I couldn't feel much change somehow. I was thinking of taking a facelift surgery if laser treatment didn't work, but now I guess I don't need to take a surgery for a while (thanks to Morpheus 8)

Be very careful with these heated lasers as they cause fat loss, dermatologists will say they don’t cause facial fat loss as it doesn’t penetrate that deep I have first hand experience……. Don’t do it

Hi Dr. Kopelman! This is the most detailed explanation of lasers I’ve encountered. You’re informative, detailed, and give a very good impression. I subscribed and would love to be treated by you. I have 2 questions. Since my teens I have had undereye depressions and dark circles. To remedy some of this, nine months ago I had undereye silicone implants placed in the area. It turned out that they are much too large for my face, so those areas are now bulky, convex, and look odd. Also, the implants make my eyes look a little smaller, one more than the other (definitely not something I was counting on). In addition, the bulkiness creates a shadow under the implants which makes it look like I have very dark circles under the bulkiness, at an area much lower than where my original dark circles were. My hope is to be able to have the silicone implants removed, but I’m afraid of doing so because I fear damage being caused to nerves or another area under the eyes. The questions I have are as follows: 1. Have you ever heard of undereye silicone implants being removed successfully? 2. If I can get them removed without mishap, what’s my best bet for treating my depressed and dark undereye area? Undereye lift? Mid-face lift that treats the area around the undereye? Lasers? Microneedling? Something else? Thank you!

Yes I have heard of patients who have had lower eyelid implants. These implants can be removed and hyaluronic acid gels like Restylane can be used to restore volume or your own fat can be a source for fat grating the lower eyelids to restore volume

I'm considering a CO2 laser treatment but have a fair amount of sun damage and freckles. (Caucasian who tans easily.) When you mention ablated lasers lightening skin with dark spots does this mean it will just remove the freckles or will it leave them lighter than my natural skin tone creating white spots? 🤔

Dr. What kind of laser , or treatment would you advice me to treat skin redness? (not due to rosasea) I had years ago when i was i teenager "dermabrasion" cause i had acne. as you know this procedure is very harsh and as result the area treated (cheeks) years, years later i still have some redness of the skin. My skin looks patchy. i am hispanic and have olive skin. (a little bit dark). Thanks God i didnt develope hyperpigmentation which i understand it can happen if your skin is a little dark. the redness has improved over the years but i would like to be able to address this problem without making it worse. unfortunately i am located in california otherwise i was going to make an appointment with you. Any advice would be appreciated. Thanks

This was incredibly thorough and helpful. I am in my 40s and have freckles that are lightening with age and retinol/sunscreen use. I would like to start either laser or micro needling treatments to firm skin… and I’d like to actually make my freckles darker. They are one of my favorite features! Would heat increase the pigment of freckles? Thanks again for your insight!

Hi doc, i just did c02 laser but my face is still swelling after 6 days and is very red, warm and flaky. I was told to apply 0.2 hydrocortisone. Is that it? Or shiuld i add a moisturiser?

Hello Dr. Kopelman, thank you so much for posting this very informative video. I’m a fair skinned, Caucasian, 55 year old woman who has struggled with the appearance of chicken pox and acne scarring on my face since I was a teenager. I finally decided to give Co2 resurfacing a try just recently. I’ve had two Co2 Resurfacing procedures done so far. Although I have seen some positive results with firmness and texture of my skin, I still don’t feel that the Co2 is making quite a substantial difference to help eliminate my facial scars. My question to you is, should I continue with the Co2 or try Microneedling or is there another treatment you can suggest? Will I eventually need surgery? I have had scarring on my face since I was a teenager before the Chicken Pox vaccine was available and I’m hoping there is help for me to be able to finally feel more confident about myself and to obtain a more permanent solution to improving the scarring on my face. Thank you in advance for your advice.

I have rather spanish skin color and got severe hyperpigmentation after having CO2 on my face. Today after 8 months of constantly using sunblock and weak peeling on weekly base, they have faded out but still visible on my cheeks. However my skin texture and pores have improved noticeably.

People of color are at significantly greater risk of hyperpigmentation following laser resurfacing therefore proceed with caution.Less is more!

An erbium fractionated treatment of your "décolleté"is a good option . CO2 lasers deliver too much heat and have the potential to cause scarring on the chest therefore avoid them
