Shoulder Dystocia: Resolution in All-Fours or Knee Chest Position (English)

This introductory video demonstrates how to position a patient in the all fours or knee chest position for resolution of shoulder dystocia. It also highlights some important points when performing fetal neck traction and internal maneuvers after repositioning the patient.
Bearing in mind that the anterior brachial plexus may have been injured by the preceding traction performed when the patient is in supine or McRobert position, one must be very cautious when attempting traction on the contralateral shoulder being stuck at the sacrum, otherwise it may end up with bilateral brachial plexus injury.
When doing internal maneuvers, the opposite hand should be inserted from above, as the maternal pelvis and the fetal facing direction are reversed. During ‘posterior’ arm delivery, its forearm should be pulled downward to facilitate rotations of he shoulders.
