What’s the cause or what type of injury it is when a person can’t rotate their shoulder internally in this position. And feels a lot of pain

This is super late, but my best guess is a subscapularis muscle tear/injury.

Did it healed up bro? And what did you do because I have the same problem , i cant raise my arm internally bended like my other arm and it causes a lot of pain and clicking , i weighlift

It was tear in the supraspinatus tendon and bicep tendon tear. Got PRP injection 2 months back. Seeing improvement with shoulder rehab exercises designed by the physio. Also, lack of muscle strength in the back and rear deltoid, which resulted in super tight pecs. So need to strengthen my back, delts and lats
