That is an engineering tinker from WOTLK, it's called nitro boosts, a belt attachment. Maybe I should have added a tooltip for them, sorry about that.

I gave it a shot many months ago before the whole lawsuit issue. It was fun but, the skill vs gear ratio was way off. From what I read, that's still the case.
