I have worked in bedside nursing as an RN doing medsurg and behavioral health for 5 years. Got burned out. I then transitioned to utilization management doing prior authorization for medical and behavioral health review for a managed care company and I have done that for 7 years. It is so stressful . I have worked from home for 5 years and I love it. I have been looking to make a switch to do something else so watching your videos was very helpful. CDI sounds amazing but I am not sure if I qualify for that since I do not have ICU experience.

Came across this video. Thank you for this information. I’m a BSN and I’m taking a CPC course and I was told by my instructor that CDI is usually the way RN’s go when getting into coding. So I’m not a critical care nurse, I’m an acute care. Is there no place for an acute care nurse in this specialty? And also, what’s a good starting point to begin this path? Im going to take a CPC exam but have no clue as to how to get where you are haha! Thanks again!

Lol at “ditch that plan”. That would all depend on what your career goals are. In order to get certified in CDI you would first need 2 years experience working as a CDI Specialist.

Hi! I am an Ob RN for more than 10 years. I already passed the CPC exam and removed my A status via AAPC’s practicode. I still find difficulty looking for a job and some of the agency wanted a FT position and with actual coding experience. Unfortunately, they dont count the practicode as an actual coding experience. I applied several times and even applied for a HEDIS position but still unlucky to get a part time job. Is there any more options that you can suggest because I am beginning to loose hope in a coding world. Thank you.

Hello Nadesha. I have an interview with a potential employer for a CDS position and was wondering what would be some things to know besides the obvious interview questions? Been an ICU RN for 5 years and got a call for an interview despite not having any CDS experience. This is a dream position for me at the moment but I don't want to come off clueless because I have no idea about anything about CCS, MediCal or any of those laws, rules regulations. Hey there, congratulations and best of luck on the interview. Some questions you can ask is 1. What systems do they use for coding? 2. What is the expected productivity once your off of orientation 3. What is the expected query rate once off of orientation? 4. How long is the orientation period being you have no prior experience. 5 . Lastly you can ask what is the relationship like with the CDI department and the physicians. This will tell you how hard you will have to work to get your queries answered. 6. Is there an escalation process for unanswered queries? 7. How often is staff Education provided for the CDS staff. I hope these were helpful.

@@naturalenursenadesha7676 from your description of the job (minus the coding part as there is a different department for that and we have logic built into our EHR that automatically codes diagnoses entered in H&Ps, procedure and progress notes), it sounds similar to my previous job. I was a utilization review nurse in the ER and my job was to recommend a level of care prior to admission to ensure the patient was placed in the correct level of care from admission. After the admission order was entered, I would then complete a review using clinical review criteria (Interqual or Milliman) and then fax to the payer. On the UR inpatient side, we were responsible for the subsequent care days in validating the current level of care and evaluating the medical necessity for the patient to remain in the hospital in that level of care based on clinical documentation, severity of illness and intensity of service. We completed concurrent reviews and faxed to payer to ensure continued stay approval. We would also deal with concurrent denials and follow the appeal process to ensure reimbursement. We would communicate with the attending regarding patients in the wrong level of care and obtain an order to either downgrade or upgrade based on medical necessity. Case management, at least for the hospital system I work for, refers to discharge planning and transitions of care. We use a triad model composed of the utilization review RN, the RN case manager and the social worker, so the roles are different but all complement each other. Does CDS involve clinical reviews at all?

I'm a cardiac rehab nurse. I do have a question, in your video you mentioned sometimes you do education to the physicians as a presentation. That is really the only thing pushing me back from this.
