Does it mean a fraction which is not in it simplest form is an irrational number? For example 2/4 is irrational??

No. Irrational numbers cannot be put in the form of fraction or a/b where b is not equal to zero. 2/4 on the other hand is a fraction simplified as 1/2, since it can be expressed as fraction. It is rational

Ther square root of all integers that are not perfect squares are irrational. Proof by contradiction is also used to show it.
