This is a good video. I'm moving from Baxter pumps to Alaris pumps. I'm finding the alaris pump a little bit difficult to learn as there so many steps. These are not user-friendly pumps. I really hate them. A lot of videos on KZbin just run through it so quickly that it's hard to learn it but this was good. Thanks

This is nice! I just graduated from my community college with an AAS in Paramedicine, and our service uses those. In school they made us calculate the rate, and drip by eye, but I never knew we were going to need these for interfacility transfers, so this makes it less scary. In my ER clinicals, the nurses really didn't feel like taking the time to explain the pump functions to me. They would mostly just use me for IV cannulation, especially in COVID rooms they didn't want to go in.

When back priming, do you need the primary clamp open? Also when administering iv meds through secondary piggy back method, do you have the primary tubing roller clamp open or closed?

Question: while the secondary is infusing, is the primary not infusing? Is it ever indicated to run the abx through its own primary tubing through its own module on the the pump and y site near the pt so that the NS is not interrupted?

Yes you can run an antibiotic as a primary. You will need two iv sites or a multilumen PICC or central line in order to administer both antibiotics and NS at the same time. You would run both as a primary to separate ports. Most often though you will run it as a secondary.

Yes.....I understand your question. The Baxter pump allows for concurrent infusion of both primary and secondary line. I've been using Baxter for 14 years now....and I am having such a hard time with Alaris as a new traveling RN

Back priming is just one of two ways to prime your secondary line. Back priming is quick and allows you to hang consecutive piggyback medications while flushing the tubing without changing the secondary line until indicated per policy (e.g. IV line expiration). When initiating a new medication and a new secondary line, you can certainly prime it like you would with your primary IV using the roller clamp method.

Yes it will as long as you remember to list it and input the correct dosage into the pump.

Back priming is just one method. But it can save time and resources (secondary tubing) when hanging the same rx multiple times throughout a shift along with other medications that are compatible with your primary fluid.

How do you know what to put for VTBI? Is that whatever is in the bag?

VTBI is the volume of the fluid/Rx or whatever is in the bag as you say, e.g. VTBI 1000 ml for 1L NS or LR (some might program slightly less than the actual bag volume to avoid running the line dry). Alaris pumps have a programmed library of various infusions that, when using the guardrails feature, will have preset dosages/volumes to select from. VTBI always needs to be programmed when using the basic infusion setting. Always follow your facility's policies and procedures.

They do have EMR interoperability with Epic (maybe others) but depends on the facility on whether that's utilized.

You must lower the primary bag if you are running a piggyback. This prevents the pump from drawing from the primary bag. Once the piggyback has infused, the primary will run even though it is still lower.

Which ever bag is higher will be the bag that is running. So if you want the medication to run you put that bag higher but if you want the primary bag to run, you put that bag higher but as a person mentioned above, once the secondary Medication is finished the primary will start to run even though its lower because they'll be nothing in the medication bag to run ( it will be empty)

The piggyback will infuse through the primary fluid line. The primary fluid will resume at its set rate after the piggyback infusion is complete. They do not run simultaneously.

A piggyback allows you to run a secondary infusion/medication through a primary line without necessitating a second IV site, so long as primary and secondary solutions are compatible (fewer lines/IV sites decreases risk of infection). Piggyback technique allows antibiotics to be added quickly to your primary infusion (e.g. normal saline) which makes nursing much more efficient when multiple antibiotics are required throughout a shift.

You do not have to loosen the end cap to prime the tubing. It is a vented endcap.
