He could have emptied a clip into a potentially non-violent or even unarmed criminal.

Just in case your comment was a genuine question and not a joke itself, as far I can tell the comedic answer to what the blurred gesture means is two-fold: first, it is an allusion to the fact that people in desperate circumstances are sometimes forced into selling sexual favours in order to make any headway, and second it is an over-the-top visual representation of an act so obscene that some (dark) meta-humour would be found in the fact that it does not pass the censorship required to be be shown to the viewing public outright. In both cases, the willingness displayed to do such an act (and the immediate level of detail with which he mimes it) indicates that Boyle's doppelgänger has resorted to such drastic measures multiple times already. At baseline level he is therefore either numb to the necessity of doing this unnameable deed, or has little pride left with which to feel disgust at this state of affairs. If you are looking for a more specific answer about the physical nature of the act portrayed, I cannot help unless it be to speculate that it appears aggressively penetrative in nature - which possibly reinforces the dark humour referenced above by belonging to [EDIT: A STILL] more obscure niche subculture, meaning that he has been brought low enough to regularly be turned away from servicing even the "normal" deviant appetites in an "I know that my standards are low, but they are still above YOU" situation. I may not have the whole picture [EDIT: NO PUN ORIGINALLY INTENDED], so anyone else can feel free to share if they want to add anything, but that seems to be the gist of things to me.
