Even in times when child marriage was normal, there was still an "age of consummation". Meaning, NO, people were not having kids as soon as they had their period. Like, start health classes young, but until the average start of puberty it's all stretching and eating healthy and what allergies are. And basic first aid. Then add in how your body changes with puberty, and what to look out for incase something has gone wrong at said average start of puberty. Then, when the statistics say about 60% of kids have started puberty, gross them out with STI's and pregnancy.

My grandmother got married at the age of 12 when she still didn't get her period. My grandfather was 22 at that time and he slept in a separate room. According to my grandmother, the first few years of marriage was almost nothing. My grandfather pretended like she was just supposed to be in his home and he was just supposed to provide for her. As she got older she got more engaged in household chores. When she got her period at the age of 14, things didn't change. Her mother in law was like a mother figure and she taught my grandmother everything she needed. They started sleeping together when my grandmother was 19 and had their first child at 24. Back in those days it was pretty common for girls to get married this early to someone her family chose. They didn't have education, didn't have technology, but still no one batted an eye when my grandfather refused to sleep with her before she turned 19. Even though her sisters were having kids at 15/16, no one pressured them for anything. My point is, even during that time, people were aware of the negative sides of giving birth at a premature age. You don't need a university degree or internet to realise that a child cannot bear another child, you need common sense. My grandmother's all other sisters had one or two dead babies during their marriage. But my mother and my 3 uncles were fine with no complications during birth.
