Actually, the science is developing to a point where Garrison's desire to get pregnant (even if it was just to have an abortion) is feasible. It would just take combining several scientific advancements we already have (though most of it has only been done on lab mice so far, they have been successful). At the current rate it's going, we are looking at a full transition including a functioning reproductive system being commercially available in about 50 years or so. I wrote my own comment explaining it if anyone wants to try and track that down

Garrison has the wildest route to learn who he is: Thinks he weren't loved as a child since his father didn't molest him. Develops a multiple identity disorder. Closeted gay who's homophobic due to his faith. Learns about his gayness, but goes crazy and exiles himself to the mountains in denial. Creates a new identity to replace Mr.Hat's closeted gayness and replaces it with Mr.Twig who rocks concentration camp gay-marker. Mr.Twig dies and he comes out of the closet and goes immediately to the extreme and starts dating Mr. Slave. Garrison then changes his sex out of nowhere to everyone and becomes homophobic again. Then he meets Richard Dawkins whom he fells in love with and throws his faith out of the window. When future Cartman reveals Dawkins how Garrison used to be man, he leaves. He counters the persians and finds herself as a lesbian now. Later on she de-transion back to man and is openly gay but a slut. Then he finally meets Rick who seems to be a good counter balance for Garrison and he seems relatively stable with him.

A couple years before this South Park episode came out I remember Tom Green aired a bunch of stuff from his surgery when he had testicular cancer. I think that was also the last episode of the Tom Green show because he said he couldn't get anything more extreme on the air. He was always trying to push the boundaries of what was allowed and airing a pretty graphic surgery was hard to top. The Tom Green show was pretty damn funny. I think my favorite bit he did was Undercutter's Pizza.

This is one of the few episodes I don’t think holds up very well, both in today's society and modern understanding of transgenderism. It highlights a decent point about the extreme side of transhumanism but it fails to address the reality of transgender individuals and how most of them actually are, especially in a time where there was less acceptance and general knowledge of trans people. Mr. Garrison has always been a caricature on extremism, which is why, for example, they point out several times that there’s a big difference between being gay and Mr. Garrison. He's an extreme case that doesn’t represent the majority of the minorities he’s a part of, which is an important distinction as people, and the media in general, tend to lump categories of people into extreme examples because they don’t know enough about those people to discern the difference. Back then, this distinction was easier to understand for gay people but not so much with trans. However, with that said, I still appreciate Matt and Trey's attempt at trying to bring social commentary to the topic.

Remember this episode is over 18 years old and while they go extreme with Kyle and Gerald, if they had copied future reality. People would seen just as ridiculous with them trying to be too real about an argument that would never come rather than being extreme satire. And we just had a teacher telling two 8 year olds in her class that they were homophobic because they did not think it was appropriate to recognize another student that self identified as a cat. I'm not sure how the teacher came to the conclusion of not accepting a another kid in their class being a different species is homophobic. But you know that it's bad when the 8 year old has to tell the teachers that sexuality and gender identity are not the same thing. I really do feel bad for actual trans people who just wants to live their normal lives. But all the oxygen is being used up by the people who have stolen what started as the reasonable LGBT before they added more letters and numbers. Not because they are those things they just know they can get special treatment for claiming to be a part of it.

From what I can guess and remember, this episode got a lot of backlash but it also went under the radar because nobody did anything about it as far as I can tell. It pretty much gave the a simple answer to the whole trans debate, Garrison is still a guy but he got surgery to look like a woman because he wanted to be happy.
