The first option can't be changed into a question IF YOU TRY TO USE "HOW LONG". BUT. there's other way to turn it into a question and that is; - Hasn't he washed his car in a long time ?.

BEEN is not only past participle for the verb BE, but also past participle for the verb GO with the meaning "to visit", "to go and return back". Therefore we use preposition TO after that participle in a sentence.

what about GO hihing/snowboarding etc.? is it correct to say Have you ever GONE snowboarding? I've heard that it's correct to say BEEN instead

Have you ever gone..? sounds good in this case. You can also ask "Have you ever tried...?" Been to = visited Have you ever been to the USA? = Have you ever visited the USA?

I think in some other class u said Have you ever drunk goat's milk is correct. Have you ever had goat's milk is technically correct. But we shd not say
