Thank you very much for the informative video! My question is, If I have a 6-hole or 6M, as you called it, hose or connector to my unit, does a 4-hole (4M) handpiece fit on it? I really appreciate your response.

Great video. Thank you both, is it really the case that the head of speed increasing hand piece is larger than air turbine?

Hi Jaz, are you still recommeding Incidental handpeices? We've had a lot of breakdowns and two instances of severe lip burns from overheating handpieces. I believe our maintenace is good

Hi Greg - I am an advocate of Chris O Connor - he's a super knowledgeable and helpful guy. I have not purchased his handpieces as I have had my own for several years before he started to sell his. For what it's worth, I am using NSK. I have also experienced this issue with an NSK handpiece (speed increasing) and I had an incident where I burned the patient's lip (I talked about it on a podcast episode and how I managed this). The reason for this was the bearings were worn and needed replacing. I would DEFO reach out to Chris? Seeing as I have experienced this just once and you have had a few instances, something does not add up here

Hi Greg, we appreciate you have had a bad experience with our handpieces and that you are upset. What you should know is we try our very best to service, repair and educate dentists to get the most out of our handpiece products. We believe they are a good product and that our offer is excellent value. They remain popular after several years on the market. Of course they are not perfect but If you can find a brand of handpieces that never breakdown (especially 1:5’s) then stick with those. Our experience is they all have issues and need regular maintainance. Regarding lip burns we would remind any dental professional that It is the dentists responsibility to check that handpieces are running properly every time they use them and by running them for a short period before use and inspecting. Also at no time during operation should a handpiece ever touch the soft tissue of the patient. Even a perfectly functioning 1:5 handpiece will heat up over a period of use and is very dangerous to run in contact with the soft tissues.

I’m still a bit confused! If you don’t have an electric mini motor and you just have a air motor speed increasing handpiece on your unit is it still ok to section teeth and remove bone? Is there still air going through the air motor? Thanks, great episode!

The optimal would be no air and external sterile water. That would require a micromotor and external positive pressure saline supply. The reality is this is not always available or practical or perhaps proportionate to the risks involved. If you use an Air motor or indeed a micromotor and there is pressured air driving the spray then this is not the ideal situation and is a compromise. The goal of this episode was to inform dentists about the way handpieces work and the variety of options available. I think for absolute guidance on what can and can't be used we would need to look for position statements from Royal Colleges or notified bodies. I'm not aware of any consensus I'm afraid so I cannot advise.

What is the optimal RPM for caries removal with round steel burs? Often not discussed in every day dentistry. I normally change my settings depending on the case. Wondering if there is an optimal range in the literature?

Thanks Graham! I'm not sure what the academic answer is, but I can share with you what my old restorative consultant taught me (professor of Restorative Dentistry) - he told me to use rosehead burs 10-15k (if softer caries then 10, if firmer then 15) but I always use it WITH water to reduce heat. If anyone reading this wants to share your protocols or evidence base, always happy to learn!

@@protrusive thanks jaz! The range i use is 7-10k but I'm not using water, instead taking intermittent breaks using caries dye etc. Nice video👌
