Pain was called Paw fruit back in the day when Pain wasnt reworked yet and the Pain or the paw fruit and watch the anime because you will see that Bartholomew Kuma the guy who ate the paw paw fruit took all of luffy's pain out of his body using his devil fruit and gave all that pain to zoro because Kuma gave him a different option. He would allow Luffy to live if Zoro agreed to tskr all the pain Luffy had accumulated during the fight, along with all the pain Zoro already had from his own fights and zoro felt so much pain but still standing also kuma is a marine :)

The reason pain is so hard to use is bc of the aoe and the hitbox (both being quite small apart from pain C) the travel speed is also a factor of why it’s so hard to use for PvP
