Actually Moore didn’t “elect” the surgery. It was a requirement of his contract the refusal of which would have seen him likely immediately released. How can I know that? Because that’s what Houston did to their badass closer Roberto Osuna when he opted not to have the surgery and take the “rest and rehab” approach. The Astros were paying Osuna 10 million dollars a year for that arm and were paying several million more in support staff for it. It’s crazy to think that you can sign a contract that essentially gives your employer rights to determine your medical care but, that’s MLB for ya;)

Tanaka never had Tommy John surgery. He opted to do rehab and never needed it. He basically continued to pitch on a string of an elbow ligament and was the ace of the Yankees pitching staff while doing it. IMO he was definitely worth his contract. Also; Tommy John surgery has success rate in the 80%ish range for major league pitchers. I do understand however that less than that come back with the same stuff they had before. However; Jacob DeGrom, Jose Fernandez, Luis Severino just to name a few have all recently had the surgery and have come back with no discernible difference in their pure pitch effectiveness. They all still have their velocity, spin, and break.

The problem has been there for decades. Pitchers with filthy stuff are told in the minors that they have great mechanics and that they'll have long careers when, in reality, they're obviously destroying their arms. Teams just gamble on getting a few good years out of young pitchers. After that, it's borrowed time. Maybe they can trade them before the damage catches up and get a little more for their investment...maybe not...fix the issue by cleaning up the mechanics and putting less emphasis on certain pitches and you lose the filthy stuff...
