Stallone's mother had a c-section and he suffered nerve damage to the side of his mouth. He had discussed the cause on multiple occasions.

Stallone has a wonky mouth due to a condition where the muscles in his face around the mouth didn't develop equally, so one side of his mouth can move more than the other, I believe he was also born with the side of his tounge was fused to the inner of his mouth causing a speak impediment, till he had it fixed with surgery. But the guy from 300 probably just watched Rocky as a kid, and thinks it's cool to talk like that.
